Application

Here you will find everything you need to apply for the UTA Full Service Card for your company.

And this is how it works:

The application consists of 2 forms and one page of terms and conditions, available here as an Acrobat file. These PDF files can be downloaded and completed directly using Acrobat Reader.

Unfortunately, we cannot accept direct applications by e-mail because of legal reasons. Both parties need to sign the application and the terms & conditions.

Please print the completed forms in triplicate and send us 3 sets with original signatures by normal mail. We will send back a signed version for your records

Steps:

1. Download new customer application (PDF)
2. Fill in forms online
3. Print forms in triplicate
4. Please sign all three versions of the application, the terms & conditions, and the PIN-Code-statement.
5. Send everything via normal mail  to the following address:

You will receive back a signed version of the application and the terms & conditions for your records.
